Position Description

The Visiting Nurse Association is currently seeking a detail oriented, self-motivated registered nurse who enjoys making a difference in the lives of young families for our Love and Learn Teen and Young Parent public health home visitation program.
This Full-time or part-time employee will work in client homes and in the community providing services to pregnant or parenting teens and young families enrolled in the Love and Learn home visitation program. They will use the nursing process to develop a plan of care for families enrolled in the program who are identified as needing a focus on health-related goals for one or more family members. This employee will also work with families to improve parenting skills and child outcomes utilizing the interventions of education, support, and community referrals. This employee will receive certification in the international early childhood curriculum “Growing Great Kids™” while employed in this program.

Typical duties include:
• Completes maternal and child health assessments, health education and referrals to health care resources.
• Working with the parent coach to assess family needs and develop a plan of care in collaboration with the family.
• Participates in multi-disciplinary team case conferencing with partnering agencies.
• Implements interventions in home visits that are family-centered, strength-based and directed toward improving family outcomes.
• Initiates appropriate preventive teaching and advocacy for families.
Requirements:
• Graduation from an accredited school of nursing. A Baccalaureate degree in nursing is required.
• Currently certified in Basic Life Support.
• 1 year of nursing experience required. Experience with maternal or child nursing preferred.
• Ability to maintain flexibility in schedule with some early evening or weekend visits possible.
• Valid Driver’s License and Auto Insurance coverage.
• Must have your own reliable transportation.
• Bilingual in Spanish preferred, but not required.
